By using the Site (including booking, messaging, downloads, and any patient portal links), you agree to these Terms of Use and our posted policies. If you do not agree, do not use the Site.

1) No Medical Emergencies or Specific Medical Advice

The Site provides general information only and is not a substitute for in-person examination, diagnosis, or treatment. Do not ignore or delay seeking care because of something on this Site. For urgent issues, call 9-1-1 or go to the nearest emergency department.

2) Provider–Patient Relationship

Using the Site does not create a provider–patient relationship. A relationship is formed only after: (a) you complete intake/consent, (b) your appointment is accepted, and (c) you are seen by a clinician licensed in your state.

3) Telehealth Notice (High-Level)

If you choose virtual care, you consent to receive services via audio/video/data technologies and acknowledge typical telehealth risks (e.g., technical failures, limited exam). Full telehealth consent will be presented during intake and must be accepted before care.

4) Direct-Pay / Insurance

At this time, Taylored Medical currently operates as direct-pay. We do not bill insurance. You are responsible for fees at booking. You may request a receipt (superbill) to submit to your insurer at your discretion; reimbursement is not guaranteed. See our Patient Financial Policy for details.

5) Privacy & HIPAA

Your health information is protected by HIPAA once a provider–patient relationship is formed and you use our clinical systems. See:

6) Appropriate Use of the Site

You agree not to: (a) upload unlawful, threatening, defamatory, or harmful content; (b) attempt to access unauthorized systems; (c) use bots/scrapers; or (d) interfere with Site operation. We may suspend or terminate access for violations.

7) Intellectual Property

The Site and its contents (text, logos, graphics) are owned by Taylored Medical or its licensors and are protected by law. You may view and print pages for personal, non-commercial use only. No reproduction, distribution, or derivative works without prior written consent.

8) Electronic Communications & Messaging

By contacting us through the Site, you consent to receive replies electronically (email/SMS). Message/data rates may apply. Do not transmit sensitive medical details through general contact forms; use the patient portal or secure intake when provided.

9) Third-Party Links & Tools

The Site may link to third-party sites or services (e.g., video, payments, labs). We are not responsible for third-party content, availability, or security. Use them at your own risk and review their terms and privacy policies.

10) Disclaimers

The Site is provided “as is” without warranties of any kind (including accuracy, completeness, fitness for a particular purpose, availability, or non-infringement). We may update or change content at any time without notice.

11) Limitation of Liability

To the fullest extent permitted by law, Taylored Medical and its clinicians shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of the Site. Our aggregate liability for any claims related to the Site shall not exceed $500.

12) Governing Law and Venue

These Terms are governed by the laws of the State of Michigan. Exclusive venue for any permitted court action is in the state or federal courts located in Grand Rapids, MI.

13) Changes

We may update these Terms at any time. The “Effective date” will reflect the latest version. Your continued use of the Site after changes means you accept the updated Terms.
