Taylored Medical is required by law to maintain the privacy of your protected health information (“PHI”), provide you with this Notice, and follow its terms.

Restrictions: You may request limits on how we use or disclose PHI.
Alternative Communications: You may request PHI be sent to you in a specific way or location.
Inspect & Copy: You may view or request copies of your PHI. A reasonable fee may apply.
Amendments: You may request corrections to your PHI.
Accounting of Disclosures: You may request a list of disclosures we’ve made.
Copy of Notice: You may request a paper copy of this Notice at any time.
How We Use & Disclose PHI
We may use or disclose your PHI without written authorization in the following ways:
Treatment: To coordinate and manage your care.
Payment: To bill and obtain payment for services.
Healthcare Operations: To support quality improvement, training, compliance, and audits.
As Required by Law: For judicial, administrative, or law enforcement proceedings.
Public Health & Oversight: For disease control, reporting, and regulatory audits.
Coroners & Funeral Directors: As needed to fulfill their duties.
Research: In limited circumstances and with protections.
To Prevent Serious Threats: If necessary to protect health or safety.
Special Government Functions: For military, security, or intelligence purposes.
To You: You may access your PHI directly.
To HHS/OCR: For HIPAA compliance investigations.
Other Uses
Any other use or disclosure not covered by this Notice will require your written authorization. You may revoke an authorization at any time in writing.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services. You will not be penalized for filing a complaint.
